| FORM NAME | FORM DESCRIPTION |
| Interp. Tape Agreement | Interp Tapes (final round video tapes from nationals not included). If you are a District Chair, complete and file the attached Interp. Tape Agreement form with the NFL office. |
| Supply Requisition Forms | Excel file consists of three worksheets that can be filled in and emailed to the NFL. You must click on 'Enable Macros' to open Instructions and Excel File |
| District Tournament Report | The District Tournament Report is used to record decisions and ranks awarded contestants for an individual school in a district tournament. This is 2 page fill-in form. Each report for an individual school must be signed by the district chair after printing. |
| District Tournament Qualifier/Alternate Form | National Qualifier Form is sent to the National Office at the conclusion of a district tournament. Signatures required. This is a two page fill-in form. |
| Nationals Semi-Final & Final Judge Requirements Form | Complete this form to specify the semi and final round judges your NFL District will furnish at the National Tournament. Must be received at the NFL Office by May 1, 2008. This is a fill-in form. |
| District Score Sheet | The Score Sheet is used to tabulate the judges' ranks |
| Final District Score Sheet | The Final Score Sheet is used to tabulate judge ballot in the final contest. 11 07 |
| Results Form | Results Form is to be used for each contest round |
| Assignment Sheet | This form is for listing the contestants in the order in which they are to speak and the judges for that section. |
| Policy Debate Judge Philosophy Card | This original form is used to obtain paradigm information from judges assigned to Policy debate. 02/28/08 |
| LD Debate Judge Paradigm Card | This original form is used to obtain paradigm information from judges assigned to Lincoln-Douglas debate. 02/28/08 |
